Collaborative Projects and Programs

 

The Clearfield County Collaboration and Prevention Board has adopted the CTC risk and resource assessment process as the basis for annual “strategic planning.”  As a result, the Board has actively supported implementation of CTC promising approaches and other research-based programs in order to address risk factors identified during strategic planning. Significantly, such programs have been implemented by diverse member agencies, rather than any single agency implementing the bulk of the programs. At times, such programs have been implemented primarily by individual agencies with the support of the Board. At other times, programs have been implemented on a collaborative basis with multiple agencies assuming program responsibilities. Representative examples of collaborative projects developed during the Board’s history include the following (please note that not all projects are currently ongoing):

 

  • School Based Mentoring – Funded by Communities that Care® implementation funding, this program strengthens and supports a stable family foundation.  High School mentors work with elementary students and their parents to prevent the emergence of problem behaviors.  School-Based mentoring is coordinated through the Children’s Aid Society in partnership with local school districts.
  • CARD (Collaborative Approach to Reducing Delinquency) Program – A collaboration of five agencies (Children’s Aid Society; Clearfield County Office of Children, Youth and Families; Gateway Institute and Clinic; Clearfield County Department of Probation Services – Juvenile Division; and Cen-Clear Child Services, Inc.) to develop a delinquency prevention program for youth age 10-15.
  • PATHS (Promoting Alternative Thinking Strategies) – Promotes emotional and social competencies by reducing aggression and behavior problems in elementary school children while simultaneously enhancing the educational process in the classroom.  PATHS was implemented by a partnership of the Clearfield Area and Philipsburg-Osceola Area School Districts .
  • Project S.U.C.C.E.S.S. (Schools United in Clearfield County to Empower Students to Succeed) – An after-school program representing a consortium of all eight school districts in the County.  Program features have been designed to target academic difficulties; antisocial behavior;

  • P.C.H.P. (Parent-Child Home Program) – A parenting and early literacy program designed to strengthen verbal and educational interaction between parents and children age 2 and 3 in a home-based setting. P.C.H.P. was implemented by Cen-Clear Child Services, Inc.
  • F.A.S.T. (Families and Schools Together) – This program uses a multi-family group approach to strengthen childhood outcomes and has been used for the prevention of substance abuse, juvenile delinquency, school failure, abuse and neglect, and mental health issues. It was implemented by Children’s Aid Society in several county school districts.
  • SAVE  (Supervised Access for Visitations and Exchanges) – This program, implemented and operated by the Clearfield County League on Social Services, provides safe, family-friendly sites for supervised visits and exchanges of children between custodial and non-custodial parents.  A parenting curriculum is also provided to encourage effective co-parenting skills. Collaborating partners include the Clearfield County Office of Children, Youth and Family Services; the Domestic Relations Section; Children’s Aid Society; the Crossroads Project of Clearfield; the Clearfield County Courts; and local police departments.
  • Nurse Home Visitation Program – This program uses nurse home visitors to improve the health and social functioning of low-income first-time mothers and their babies. It was implemented in Clearfield County and surrounding areas by Peritech Home Health
  • Roller Coasters Program – Designed for children who are experiencing major changes in their lives, most notably the divorce of their parents. Roller Coasters was implemented by the Clearfield County League on Social Services, Inc

 

  • Clearfield-Jefferson Drug Free Communities Support Coalition – This group is actually a joint committee of both the Clearfield County Collaboration and Prevention Board and Jefferson County Communities in Collaboration for Families and Children. Its purpose is to address substance abuse in the bi-county area by assessing the scope of the problem and developing strategies to reduce substance abuse among youth and adults in the local population.
